Use this form only to report the scores of games with Play-By Dates.

Match Information
To submit scores, ensure only the designated player reports the scores. Use the first box in each column for the first set, the second box for the second set, etc. If you report a score with only a final score, use the first box only (e.g., for billiards, if a player won 3 games to 2).
